Health insurance helps protect you from high medical care costs. Many people in the United States get a health insurance policy through their employers. In most cases, the employer helps pay for that insurance. Insurance through employers is often with a managed care plan. These plans contract with health care providers and medical facilities to provide care for members at reduced costs.

You can also purchase health insurance on your own. It usually costs you more than employer-based insurance. If you do not have health insurance, you must pay your medical bills directly or rely on health care providers or organizations that donate care.


Understanding What It Covers

What are "covered services"?

Your health insurance policy is an agreement between you and your insurance company. The policy lists a package of medical benefits such as tests, drugs and treatment services. The insurance company agrees to cover the cost of certain benefits listed in your policy. These are called "covered services."

Your policy also lists the kinds of services that are not covered by your insurance company. You have to pay for any uncovered medical care that you receive.

What is a medical necessity? Is that different from a covered service?

Keep in mind that a medical necessity is not the same as a medical benefit. A medical necessity is something that your doctor has decided is necessary. A medical benefit is something that your insurance plan has agreed to cover. In some cases, your doctor might decide that you need medical care that is not covered by your insurance policy.

Insurance companies determine what tests, drugs and services they will cover. These choices are based on their understanding of the kinds of medical care that most patients need. Your insurance company's choices may mean that the test, drug or service you need isn't covered by your policy.

What should I do?

Your doctor will try to be familiar with your insurance coverage so he or she can provide you with covered care. However, there are so many different insurance plans that it's not possible for your doctor to know the specific details of each plan. By understanding your insurance coverage, you can help your doctor recommend medical care that is covered in your plan.

* Take the time to read your insurance policy. It's better to know what your insurance company will pay for before you receive a service, get tested or fill a prescription. Some kinds of care may have to be approved by your insurance company before your doctor can provide them.
* If you still have questions about your coverage, call your insurance company and ask a representative to explain it.
* Remember that your insurance company, not your doctor, makes decisions about what will be paid for and what will not.

What happens if my doctor recommends care that isn't covered by my insurance?

Most of the things your doctor recommends will be covered by your plan, but some may not. When you have a test or treatment that isn't covered, or you get a prescription filled for a drug that isn't covered, your insurance company won't pay the bill. This is often called "denying the claim." You can still obtain the treatment your doctor recommended, but you will have to pay for it yourself.

If your insurance company denies your claim, you have the right to appeal (challenge) the decision. Before you decide to appeal, know your insurance company's appeal process. This should be discussed in your plan handbook. Also, ask your doctor for his or her opinion. If your doctor thinks it's right to make an appeal, he or she may be able to help you through the process.

* Uninsured Motorist - Pays your car's damages when an auto accident is caused by a driver who doesn't have liability insurance.

* Underinsured Motorist - Pays your car's damages when an auto accident is caused by someone who has insufficient liability insurance.

* Rental Reimbursement - This type of coverage will pay for a rental car if your car is damaged due to an auto accident. Often this coverage has a daily allowance for a rental car.

Many insurance policies combine a number of these types of coverage. The first step in choosing the insurance you want for your car is to know the laws in your state. This will tell you the minimum insurance you need for your car. It's good to keep in mind that, just because your state may not require extensive insurance, extra coverage may be worth the expense. Convert HTML for Adsense Code


This article will inform you how to make the code compatable in XML Blogger Template. To use AdSense code inside a post it is necessary to "escape" the AdSense code. The reason: If you put the code into your template just as it is written when you display the source code for the page, the Google data is invisible...that does not make Google happy and it is a TOS violation of AdSense...and they will pull your account for that!

Adsense does not allow you to alter their code other than "minor" edits. what we are going to do is considered a "minor" edit.

I contacted AdSense with the following question:

Subject: Wrapping Adsense code in blog post Date: Wed, 18 Apr 2007 09:12:15 -0700
"I would like to put the adsense ads inside of a blog post. To make it look EXACTLY like you give it to me (when viewed as a Page source code) it is necessary for a minor alteration. For Blogger to parse the Adsense Code in XML template, the Javascript has to be escaped, using html entities...It looks EXACTLY like you gave it out when viewed by Page source, so is this permissable?"


Adsense answered my email with this:

"Hi Bonnie,
We do permit minor edits to the AdSense code to place inside the blog posts. Please keep in mind publisher may not alter any portion of the code or manually change the layout of the ads. For instance, publishers may not alter the ad height variable to mask the Ads by Google moniker.

Have you ever tried to insert the Google Adsense code directly into your Blogger XML Template and get this error?

Banner Rotation Code - Magic Image Rotation

Magic Image Rotation is a very simple image rotation script. This script functions much like a slideshow, but has the added feature of allowing each image or banner to have unique links associated with them. All this is done without the need for annoying page refreshes.

This script is free to use as long as the copyright notice remains unchanged and intact. I do request that you send me a link so that I may see it in use. I hope this was what you were looking for.

How to use :

Unlike a lot of banner rotation scripts, you are not restricted to one banner size within a given space. So if you want to rotate both 120×240 banners and 120×600 banners in the same space, you are free to do that.

You can also select how many seconds the banners will display and there is an optional text description for each banner.

One feature I wish it had is the option to shuffle the banners. You must show the banners in the same order every time the page loads. The only way to change the order is to manually change them in the script.

That’s really the only downside to Magic Image. It’s very easy to setup and I’ve provided the instructions below

1) Download it.
2) Edit the magicimage.js file (there are instructions in the script)
3) Upload the banners and magicimage.js file to your server
4) Add the following lines of code inside the <head> tag of your page.

3) The author is "lazy people trying to get money even easier"

Therefore,
1) Never to download the 'cheat' program, because the program is a Trojan or Worm or something like that. Once you download a little "exploit" the program, chances are that's either a Keylogger or Spyware, and before you know it, he has free access to some your accounts in a PTC-site and maybe in Paypal or Alertpay.
2) When you were found out did dishonest, your account banned or suspended because Admin could checks the logs or audit and sees those pitiful hack attempts.

How Google crawls, indexes, and serves the web

When you sit down at your computer and do a Google search, you're almost instantly presented with a list of results from all over the web. How does Google find web pages matching your query, and determine the order of search results?

In the simplest terms, you could think of searching the web as looking in a very large book with an impressive index telling you exactly where everything is located. When you perform a Google search, our programs check our index to determine the most relevant search results to be returned ("served") to you.

The three key processes in delivering search results to you are:

Crawling

Crawling is the process by which Googlebot discovers new and updated pages to be added to the Google index.

We use a huge set of computers to fetch (or "crawl") billions of pages on the web. The program that does the fetching is called Googlebot (also known as a robot, bot, or spider). Googlebot uses an algorithmic process: computer programs determine which sites to crawl, how often, and how many pages to fetch from each site.

Google's crawl process begins with a list of web page URLs, generated from previous crawl processes, and augmented with Sitemap data provided by webmasters. As Googlebot visits of each these websites it detects links on each page and adds them to its list of pages to crawl. New sites, changes to existing sites, and dead links are noted and used to update the Google index.

Google doesn't accept payment to crawl a site more frequently, and we keep the search side of our business separate from our revenue-generating AdWords service.

Indexing

Googlebot processes each of the pages it crawls in order to compile a massive index of all the words it sees and their location on each page. In addition, we process information included in key content tags and attributes, such as Title tags and ALT attributes. Googlebot can process many, but not all, content types. For example, we cannot process the content of some rich media files or dynamic pages.

Serving results

When a user enters a query, our machines search the index for matching pages and return the results we believe are the most relevant to the user. Relevancy is determined by over 200 factors, one of which is the PageRank for a given page. PageRank is the measure of the importance of a page based on the incoming links from other pages. In simple terms, each link to a page on your site from another site adds to your site's PageRank. Not all links are equal: Google works hard to improve the user experience by identifying spam links and other practices that negatively impact search results. The best types of links are those that are given based on the quality of your content.

In order for your site to rank well in search results pages, it's important to make sure that Google can crawl and index your site correctly. Our Webmaster Guidelines outline some best practices that can help you avoid common pitfalls and improve your site's ranking.

What is Traffic Exchange?

A traffic exchange is a type of website which provides a service for webmasters in exchange for traffic. It is similar to the autosurf concept with the exception that traffic exchanges usually use a manual rotation.

Concept

A traffic exchange website receives website submissions from webmasters that join traffic exchange networks. The person who submitted the website then has to browse other member sites on the exchange program to earn credits, which enable their sites to be viewed by other members through the surf system. This increases the number of visitors to all the sites involved.

Exchanges enforce a certain credit ratio, which illustrates the amount of websites the surfer must view in order to receive one hit through the program for their promoted website. Many sites offer the ability to upgrade one's membership level for a more equal credit ratio.

As the viewers are all website owners or affiliates, it is possible that some might find certain member sites interesting and thus make note of them on their own sites, sending more traffic their way. Most traffic programs also impose a time limit when members are browsing, ranging from 10 seconds to 60 seconds. Some incorporate the use of captcha to ensure user interaction.

Almost all traffic exchange programs are free, although many of them offer special features to paid members and offer credits for purchase. Almost all traffic exchange programs encourage users to build their own referral networks, which in turn increases the referrers' amount of credits.

The traffic generated in a traffic exchange can be leveraged by using a downline builder to assist the user in building a referral network in the many different traffic exchanges.

In practice, traffic exchange programs are generally used by small business owners or marketers who either want free advertising or use the exchange programs for low-budget advertisement campaigns.
